To apply to the Dance Department at ASFA, you must be a rising 7th—11th grader.
Application Deadline: Student Application, Registration Fee, Parent Questionnaire, Three Teacher Recommendation Forms, School Information Form including transcript and testing information must be received on or before February 1, 2013.
Student Application: You may download the Student Application here, pick one up from the school, or request that an application be mailed to you. Once you have completed the application return it to:

Submit, mail, or email the Student Application immediately. Do not wait until the deadline to send it in. This helps us get in touch with you if we have questions about your application.
Application Fee: $25.00 non-refundable application fee must be received before the deadline. Check or money order is to be made payable to ASFA and please print student’s name in the bottom left corner of the check.
Parent Questionnaire: Form must be completed and signed by the parent and returned prior to the deadline.
Teacher Recommendation Forms(3): Forms must be signed by the parent and student before giving to the teacher to complete. The forms must be completed by your current English teacher, a dance teacher, and a teacher of choice. Give these forms to the appropriate teachers with self-addressed-stamped envelopes (address above). These must be received before the deadline.
School Information Form: Form must be signed by the parent and student before giving to the Principal or Guidance Counselor at your school to complete. This form and all of the requested information must be completed and returned to the address above on or before the deadline.
Performance/Study Experience/Resume: You may complete this form or complete the top section and attach your resume to it. This form needs to be received prior to the deadline.
Only completed applications received on or before the deadline will be reviewed. It is the applicant’s responsibility to call and check on the status of your application.
